Site Partner Application Requirements

Documents and information needed to become a partner site

Hospital Information

Hospital name, complete address, contact numbers, and administrative details including registration certificate and accreditation documents.

Registration Certificate

Valid hospital registration certificate and any other relevant accreditation documents from regulatory authorities.

Investigator Credentials

CV and certificates of potential Principal Investigator including medical qualifications, clinical trial experience, and therapeutic area expertise.

Laboratory Details

NABL accredited local laboratory details with certificates and laboratory normal ranges list for various diagnostic tests and assessments.
